Runner's Dystonia: The Rare Condition Every Runner Should Know About

Most running injuries involve the muscles, tendons, or bones. But what happens when your leg suddenly stops moving the way it should—even though nothing hurts?

For a small number of runners, the answer may be runner's dystonia, a rare neurological movement disorder that is often mistaken for a running injury. Because the condition is uncommon and difficult to recognize, many runners spend years searching for answers before receiving the correct diagnosis.

What Is Runner's Dystonia?

Runner's dystonia is a task-specific focal dystonia, meaning it only occurs during a specific activity—in this case, running. Rather than being caused by an injury to the muscles or joints, the condition originates in the nervous system. The brain sends abnormal signals to the muscles, causing them to contract at the wrong time and disrupting the normal running stride.

The condition most commonly develops in experienced distance runners and may affect only one leg. Interestingly, walking, hiking, or cycling may feel completely normal, while the symptoms appear only when running.

What Are the Symptoms?

Runner's dystonia can look different from person to person, but common signs include:

  • A foot that drags or catches the ground

  • Toes that curl involuntarily

  • One leg crossing toward the midline while running

  • An ankle or foot that turns inward unexpectedly

  • A feeling that one leg "won't cooperate" despite normal strength

Symptoms often begin subtly and may only appear after several miles or during harder efforts. As the condition progresses, they may occur earlier in a run or become more noticeable.

Why Is It Often Misdiagnosed?

Because runner's dystonia is so rare, it is frequently mistaken for more common running injuries or biomechanical problems. Runners may be treated for tendon injuries, muscle weakness, nerve compression, or chronic compartment syndrome without lasting improvement. Imaging such as X-rays or MRIs is often completely normal because the problem is neurological—not structural.

A diagnosis typically requires evaluation by a neurologist who specializes in movement disorders, along with careful observation of the runner's gait.
